How do I tether my iPad mini 4 to a Samsung galaxy s3?

Hi. I can turn on portable hot spot on my galaxy s3. I can see it on my iPad but when I enter the password it won't connect. I don't have any "allowed devices" yet on my s3 but not sure if I have to do that and if so, what is the device name for my iPad and s3 also asks for a "Mac" number. Where do I find the device name and number if in fact I need to complete that step?
