100% = 0db?
Does having the volume slider in iTunes all the way to the right equal 0db? As opposed to amplifying the signal with +4db, for example.

I believe setting the volume to 'max' does set output to about 0dB. You can increase or reduce this using the equalizer (Window > Equalizer).
