Hit the plus tab to create a new sheet, you should get an option to add new sheet or form. Select form and then choose the table to base it off of.

What's the trick?

Hello,

Honestly I was having the same problem trying to figure out how to make a form from my own data instead of making one from an existing template. I even called apple and they sent me to someone who worked on the Iwork suite. Even that guy had no idea what I was talking about- his answer was "well, you have to make formulas to reference your own cells- you've used excel haven't you?" I was like, "if this a touted feature to make data entry easier, it shouldn't take me entering a ton of formulas. It's not very Intuitive and the online help section only shows 1 irrelevant answer when searching for "forms"- the built in tutorial explains nothing either.

Anyways, through messing around I found out this:
You can start with a blank worksheet.

Create a table-using the format or table pop up menu.

You must make sure the top row of your data is actually in the top row (grayed or highlighted) portion of the table/spreadsheet. Look closely, this took a while for me to notice, I actually had to highlight the row with my data headers and move it into the uppermost gray header area, as well as the columns over 1 column to the gray area.

The reason why I wasn't getting the option of "new form" when I clicked a tab was because my data was not arranged in the topmost and leftmost areas (default grey areas) to show the headers for tables.

Now, if your data is arranged correctly, holding down on the "+" tab should let you get a pop up of "new form" as well as "new spreadsheet".

If done correctly, you will now see the familiar form screen. You jut have to select which table you want it to reference, and it will now create "pages" in your form based on the number of rows your table has.

Hopefully this helps. Better yet, hope Apple updates their tutorial and help to make creating forms easier to understand. Good luck!

I agree, a pop up dialog box would be nice.

I discovered that a table can have multiple column and row headers. When you initialy create a form it will pull data from the first headers only. Any subsequent headers are for some other purpose of which I don't understand at this time. I'm currently using them to add addition descriptions for the header cells.

Here's more. If you import a spreadsheet from MS Office it is tricky to set up proper row and column headers. The table's header data must completely agree with form requirements. I'm still working on this and will try to remember to post the rules as I uncover them.
