I actually tried every method (aside from installing other external software) that you'll have stated.
I have an iphone 3GS and a windows vista, although I tried with a windows 7 computer and I couldn't upload the photos as well.
We all know very well, in the past you could just connect the iphone to the USB and then to the computer and it will be detected as a device, but now it no longer works. It never gets detected whether I have PIN code or not, whether I deleted any photos or changed autoplay settings. It is simply no longer a device/hard disk and there is no way to get the photos out except to email them one by one. Of course this can't work for me very much now, because it's been a year since I could last manage to upload my photos, and I have accumulated more than 2000 photos. I can only email myself those that I needed, but now my phone memory is reaching a limit and I need to get those photos out!
I solved this simply by installing Apple's iPhone Windows Compatibility Pack on my PC:
Yes, I know it says Windows 7 but it worked on Windows XP SP3 too.
The other really useful tip was to unlock the iphone with password - it won't recognise a locked iPhone.

i Know how to do it: plug you iphone 4 into the computer and follow these instructions, start ---> control panel ---> hardware and sound ---> devices and printers, scroll down to apple iPhone then right click ---> browse files ---> apple iPhone ---> internal storage ---> DCIM ---> whatever folder comes up click it and your pictures and videos will show up
