I save an item to the desktop and its icon does not appear on the desktop. I open a finder in the application that created the item and I can see it on the desktop in that finder.
Started happening with upgrade to Mojave.

Restarting may help. Also starting in Safe Mode then starting normally.
Restarting iMac restored items to desktop. I then saved an item onto the desktop and it shows up. Periodically, however, they stop showing up. If you google this issue, you'll see many people for several years back with the same issue.
Are you an Apple Tech. Do you know what causes this issue? It is not related to the iCloud as my iCloud was not saving documents or desktop items.
Are you saying to uncheck Desktop & Documents Folders under System Preferences ? iCloud > iCloud Drive > Options?
It wasn't checked in the first place. I have chosen not to have desktop and documents saved by the iCloud.
Any other solutions?
Well glad it's working. No I am not an Apple tech. Apple is not here. We're all users like yourself here. Personally I've had oodles of Macs over the years and I've never had that happen.
I had the iCloud drive Desktop and Documents unchecked. Just to test, I turned off iCloud Drive and saved a document again to desktop and it is not showing up. But if I open a finder through an app, I can see the items listed on the desktop.
I'm guessing if I restart the iMac they might show up. I'll let you know.
Uncheck System Preferences > iCloud > iCloud Drive > Options… > Desktop & Documents.
Or just look in your iCloud Drive in the Finder Sidebar.
Yes, that's what I was saying. Did you look in your iCloud Drive in case? I don't know why it wouldn't be visible otherwise.
