iPhone asking for PUK code

I bought my daughter an iPhone this past Friday January 18th and it worked perfectly fine until today it started asking her for a PUK code

Someone enabled a SIM PIN on the phone. If you fail to enter the SIM PIN you need to get a PIN Unlock Key (PUK) from your cellular carrier. You can usually get this by logging in to the carrier's website, or just call the carrier and ask. The alternative is to get a new SIM card and NOT enable the SIM PIN.
