  1. Lock you iPhone
  2. With the iPhone locked, swipe left to see the spotlight search suggestions and other info
  3. Scroll down to the very bottom
  4. Click on the "Edit" button
  5. Unlock your phone when prompted
  6. Now the screen should say "Add Widgets"

That's it.

Try the following. Stop if the issue is resolved.

  1. Restart your phone: Restart your iPhone, iPad, or iPod touch - Apple Support
  2. Remove the Shortcuts Widget and then re-add it. Confirm that all shortcuts are added to Widget.
  3. Save all your shortcuts. Then work through the steps here: If an app unexpectedly quits, stops responding, or won’t open - Apple Support

Not all shortcuts are automatically added to the widget "page." If you tap "manage shortcuts" in the above image you should be brought to a screen where you can check off the shortcuts you want to appear in widget. Alternatively, you can turn on "Show in Widget" from the shortcut settings in the Shortcuts app.
