Health App stepps

Un-pair the watch for one night. If that makes no difference work out what apps the phones have in common. Check Health app DataSources settings.

This is mine (jogged this morning) I have Health app and Polar watch/app. The bit at 00 is me taking the phone upstairs! The phone itself will sense movement but that 500 step flatline is something else.

Also you csn drill down into the Sources heart at bottom of screen which will show sources, then drill into each to see where the 500 steps comes from. Mine below.

Check your input sources. Apple watch??Mine shows zero all night as you would expect

Check all data it will show where the Data is being added. Ta

Good Morning, this worked. I unpaired the watch from the phone, deleted all of the history, complete start again and so far the problem has gone away.
