MacBook Pro running Mojave 10.14.3, latest software update yesterday in attempt to solve this problem.
I have restarted in safe mode, to find that headphones also disappear from sound output preferences, the only option being AirPlay.

I would test if the issue persists in a different admin user as in https://support.apple.com/en-us/HT204443
And as there seems be issues with the output sources as well, test resetting SMC (worth a shot at least) https://support.apple.com/en-us/HT201295
If issue persists after these troubleshooting steps, I would go ahead and reinstall MacOS from recovery
( https://support.apple.com/en-us/HT204904 ) If issue still isnt solved by reinstalling, erase disk before reinstalling again in order to isolate the issue to either software or hardware.

Thank you. I followed those instructions but nothing has changed.
If you have any other ideas I'd be grateful for them! I see that many other people have had the same problem with no resolution, which is disappointing.
