I am facing a similar issue with a different application:
Saved a pdf document on NOTES (iPad pro) and started taking notes with my pencil.
Saved and reopened later taking more notes in the same document.
When I have “done”, the new document saved missed part of my previous notes. Not all of them were missing.
I am working wireless on a office wireless network.

What iPad model are you using?
Your bottom signature states an iPad Mini, presumably, running some version of iOS 9.
The Apple Pencil does not work with any iPad Mini models and only works with iPad Pro models and the 2018, 6th gen 9.7 inch screen iPad.
You need to give us more accurate info about the iPad you are using.

If you are routinely marking up PDFs it is well worth investing in one of the specific PDF markup apps like GoodReader, PDF Expert, or others. They offer a great deal more capability than Notes, and are designed for exactly the task of marking up a PDF document.

There are much better note and PDF apps out there than what the very basic Apple iOS Notes app can do.
