Unknown error message pops up on my locked phone randomly.
Prompts me for a "Cancel" or "Accept" response.
Wipes after a few seconds after no user input.

Hi I had exactly the same problem and the same stupid response on the Community - I did not forget my password (do people even read the description of the problem?!)
If anyone else sees this - don't bother with Community responses. I Contacted Support directly. A great guy explained on chat that it is a really unusual error and he had not seen it before - he sought senior team leader assistance - it took them a while, and they didn't really explain what the problem was but the solution is simple :
If you don't have any outstanding updates waiting then I'm sorry I can't help you !
Same problem here, got the first error today on locked iPhone SE. 12.1.3 (16D39)
I have this error too. The replies so far keep telling me what to do when I've forgotten my password. I have not forgotten my password. Same as you - the error message just appears randomly on the lock screen !
Same here! Would like to know what the **** it is?!?!
I caught a screenshot of it on the second or third time it happened, so can’t say for sure the error code was the same on the first couple of times.
Also seeing this on an iPhone 7+ with iOS 12.1.4
