Is it possible to create a notes template, like a meeting agenda or client intake form, and then write (apple pencil) on the template?

Not with the basic iOS Notes app.

You will need to use a more more powerful and feature rich, third party iOS notes-style app to do this.

For the meeting agenda app, apps may already exist for that.

Look at apps like Things or Wunderlist or something similar.

May or may not have Apple Pencil support.

Here's is a short, incomplete list of compiled, third party iOS notes-style apps that are Apple Pencil/third party Bluetooth stylus compatible.

Pages

Bear

EverNote

INKredible

Liquid Text

Margin Note 3

Microsoft Word for iOS

Microsoft OneNote

Notability

Notepad+

NoteShelf (1 or 2)

Notes plus

Notes Writer/(Pro)

Good Notes

uPad

Penultimate

Zoom Notes

Here is what you can do with Notes in iOS.

Use Notes on your iPhone, iPad, and iPod touch - Apple Support

The ability to create a template depends on how sophisticated you wish to be. For example, if you're simply going to create a checklist, you can create one in Notes and then copy and paste it to a new note to create a similar checklist.

If you want something more sophisticated, consider using Apple's Pages application.
