Are your movies stored on an external storage device, like a NAS? Or is your library the one managed by iTunes by default?
I know I've had duplicates show up when re-adding to my library files that are on my NAS, but only when I select to re-add the entire drive, i.e. the root directory. iTunes usually does a good job of skipping the duplicate entries, but in my experience I've noticed duplicates show up after I've taken my laptop outside of the network where the NAS lives.
Although it may be unrelated to your specific issue, I've found it best to add folders and files individually to the library instead of adding entire directories of content that may/may not already be in the library.

MovieA was pointing to the iTunes Media folder/Movies/MovieA file. But funny, I also had a home video entry named MovieA pointing to the iTunes Media folder/Movies/MovieA file.
I fixed the problem by first deleting the MovieA entry in the Movies catalog, and second, by deleting MovieA in the Home Videos catalog.
The problem seem to be fixed ... but I have no idea how it was created.
