How can you download a App, which is not available in your country?

Hello,

I want to download a App, which is not available in Czech, but in America (for example)

People say I would be able to get the app, If i switch my Apple-ID/Account (create a new one in the available country)

But don’t I have to enter a Payment-Method?

(The App is free by the way, but in-app-purchases)

If there would be any other way to get the App without entering a Payment-Method, I would be really glad!
