This is very challenging
someone calls my phone and my wife phone rings at the same time
all settings are off on WiFi calling and call forwarding and other calls etc
still it’s happening

Calls on Other devices (Is not Wifi Calling). But it will work when on Wifi (Voice Calls)
Settings > Phone > Calls on Other Devices > [Select the devices]

Did all of what? Do you share an Apple ID with your wife? If you do (a bad idea) that is why.
Do you share your Apple ID? This can be the reason. Go to Moblie data settings
