I am unable to set up SMS forwarding on my phone to my car. I have one device (iPad) set up and working, but cannot find a way to add a new SMS forwarding device.
iPhone 7, iOS 12
The phone pairs with my car without any problem. I can make and receive calls, and play music from my phone on my car radio. Text messaging is the only thing that doesn't work. The car display says SMS connection failed.

Have you checked your car's manual? Most vehicles I've seen "pair" with a phone so they can show notifications on the car's interior display.

While the phone is connected to the car, go to Settings>Bluetooth
Select the i next to the device. See if there is an option to allow notifications.
That's an issue with the car, then. See if you can update the head unit yourself or if you have to bring it in to a dealership for help.
