Is there a way to add notes or keywords / tags to photos on an iPhone using Apple’s photo app?

Photos for iOS on the iPhone cannot display keywords, titles, caption. So it has no tool to add them.
But if you are syncing the photos with iCloud Photos from a Mac, you can add keywords, titles, caption in Photos on the Mac, and then use these metadata in searches on the iPhone.
You can install third-party applications like ViewExif and Metapho or Investigate. You can call them from the Share Menu in Photos on the iPhone and view the metadata added in Photos on the Mac, including many more exif tags, that Photos is keeping hidden from the user. The paid version (3.99€) of Investigator (The Photo Investigator on the App Store) will allow you to edit the locations, captions, the time stamp directly on the iPhone or iPad.
Alternately use the Markup extension, that is already installed on the iPhone. with Markup you can write text as a graphics overlay directly on the photo. (https://help.apple.com/iphone/11/#/iph893c6f8bf)

You may want to send a feature request to Apple. perhaps we will get this option back, if enough users request this feature.

Tagging photos with keywords and titles has been possible in previous versions of iOS with iPhoto iOS. That great app does no longer run on the newer system versions since Photos iOS came out.
The 3rd Party Apps are helpful. Still very surprised this is not part of the photos app.
