No place to enter Apple ID verification Code

I am trying to reconnect my Macbook Pro (OSX 10.9.5) to iCloud. After I put in my password, it sends a verification code to my phone. Then the window on my Macbook asks me to enter my password followed by the verification code. But there is no text box anywhere to enter this. Any idea WHERE I am supposed to enter my password and the verification code?
