The form for "Complete your Apple ID" malfunctioning

The app store redirected me to review my account from the app store and at the point where I was to enter payment information and submit, there was an unstated error. It repeatedly states "You did not complete the entire form" when i have filled out every possible box.

App Store error when you try to download an application from the app store, it gives a verification error, asks for an apple id password, enters an error and a window opens with a payment method, asks to enter a card, then again an error

contact iTunes Support. Either use the form or scroll to the bottom of the page in the 2nd link. Follow through the screens to get a chat or telephone option.
