SOI Removal login error - Can't get rid of U2!!

Hi I am tying to remove Songs of Innocence from my itunes account but cannot login in via the SOI Removal page. I have 2 factor auth enabled and based on previous threads the solution was to add the "2FA-Code" after my password. This results in a "incorrect password" notice from the login page for me. If I put my regular password I get prompted by the 2 factor auth, when I enter that code after my password I get no error message from the SOI Removal page but it simply returns the login prompt.

I can verify that my user name and password is correct as I was able to login to the apple site no problem.

Any other suggestions??
