Old Phone transfer difficulty

I gave my sister my old Phone (IPhone 5). I cleaned it and received unlock permission from ATT, my current carrier. My sister uses Consumer Cellular as her carrier. When she tried to get a SIM card for the phone from Consumer Cellular, they said that the phone is still associated with my account in iCloud. I checked and the phone is not listed. I had ATT check and they did not find the phone associated with my account. Is there a problem with Consumer Cellular? Is this an Apple problem? She lives a long distance from me, and it is difficult for me to try to figure out what is going on. Any suggestions or opinions? She could use a phone that actually works. Thanks for any information

I followed directions to wipe out as much information as possible.

That is not the same as erasing the phone and setting it up to be reactivated.

When I said "clean (the iPhone)" I followed directions to wipe out as much information as possible. When I received permission from ATT, the instructions were pretty clear. Since my sister used Consumer Cellular and not ATT, the guidelines said that all that is needed is to replace the SIM card with the SIM card from the new carrier. My sister took the SIM card that was in the iPhone 5 and destroyed it. When she had the new SIM card installed, the agent said that the phone is still associated with my account. However, ATT reviewed my account and found that the old phone was not associated with my account. I am stumped. I do appreciate your interest in my situation.
