Random sites I’ve never been to showing up in bookmarks and history

i always keep safari on private mode on my phone so I don’t have any search history, but I do have a few bookmarks. But occasionally a site I’ve never been to would be in my bookmarks or when I’m entering in the search bar, under bookmarks and history a random site I’ve never been to would be there although my history is empty and I don’t have it bookmarked. They’re still there even when I go to clear history and webdata. I’m just wondering if this is unusual or should be a concern.
