How can I get notifications on both my iPhone X and my I watch 4 ?

Apple Watch Series 4, watchOS 5

At the moment you do not.

The current design seems to assume if you are not using you iPhone, then the Apple Watch is where the notifications go. If you are using the iPhone it gets the notifications.
