You will have to have an Apple ID to do this, but it probably needs to be the same one that you use for iTunes.

You don't 'create' a podcast on iTunes: you have to create it elsewhere by making episode recordings and then creating a 'feed', which is a text file in a specific format which contains all the information about your podcast and its episodes: you submit this feed to 'Podcasts Connect' for inclusion in the iTunes Store and the Podcasts app.

Yes, it will. You need at least one episode, and it must be a real one, not a trailer or some sort of placeholder (because they need to assess whether you are breaching any guidelines).

You must include properly written 'itunes:subtitle' (brief) and 'itunes:summary' tags (longer) or you will be rejected and accused of submitting a 'test' podcast.

Be warned about Anchor: as I understand it they will submit it for you under their own Apple ID. This means it's effectively their podcast, not yours, and you can't access Podcasts Connect to get statistics, change the URL, or delete it should you wish to. I don't know whether they offer the option to let you have access to the feed URL to submit it yourself; if they do you should take it. If they don't I would advise looking for a resonsible podcasting service.
