The watch is added a a secondary device on your iPhone's current service plan. It is not an entirely new plan.
It will use the cellular connection whenever it is not near the paired iPhone.
The cost is the same regardless of whether its using the connection or not.
You must have a separate Cellular plan with your phone carrier. I have AT&T and I am charged an additional $10/Monthly, plus applicable taxes.
