Smart playlists not following rules (screenshot)

Very simple issue but one I'm having a hard time solving.

This is happening for a few playlists, but for the sake of simplicity I'll pick a one-rule playlist with the issue.

The first screenshot shows the rules for a smart playlist of songs with a rating of four stars. But the resulting playlist includes songs with no rating. Not ALL songs with no rating. Just some. Also attached is the info for one such song that the four-star playlist is including. Am I missing something obvious?

On other star-rating smart playlists (I have one for each star, 1-5) I'm noticing that songs do not leave the playlist when I change their rating (with live updating is turned on).
