Spoke to Apple support. I was told that external accounts are likely to be the problem. I had two Microsoft Exchange accounts and an nhs account. I have deleted all from the iPhone and cross my fingers it won't happen again
Is your phone managed by your employer? If so, the account may require you to change your passcode every 60 or 90 days.
What passcode are you talking about?

Passcodes don't expire. They are set by and only known to the user. Where is this request coming from?

Passcode Expired.

Thanks
