I've had my current iPhone for over a year. I've never had a need to sync it to iTunes until today, and it asks for me to either set up the phone as a new iPhone or to restore it from a backup from a long time ago (which I really don't want to do). If I choose to set it up as a new iPhone, is it going to erase everything on my phone?

No it will not erase your phone if you choose setup as new. Take a look at this Apple Recommendation.

Yes it will. It shouldn't be doing that. Sometime it should ask if you want to "Trust this computer" on the iPhone display. Allowing it to "restore" would wipe your iPhone.
Just want to correct myself. Definitely don't restore, but others are stating that "Set up as new" is really only setting up iTunes to allow a new device to connect to iTunes..
