Battery does not stay charged and at times freezes up

The battery does not stay charged and at times freezes up and I cannot turn the phone on or off. Also, sometimes it does not charge past 20%. How do I go about fixing this?

Make arrangements with Apple to have the battery tested. It may need to be replaced:
