You MUST pay for it every month. It is not a one time storage rental. It is an ongoing payment.

If you start using the plan, you pay for it every month. If you don’t pay for it, you lose the plan and the data. I often use this example. iCloud Storage is like renting a storage unit. You pay for it every month and you get a certain amount of storage space. If you quit paying on it, you lose the space and you have to remove your contents. if you simply up and leave, you will eventually lose the contents as well.

You will lose anything above and beyond the whatever level you download to. In other words, if you downgrade from 200GB to 50GB, you will lose whatever you have above 50GB. You can manage the data that you backup and you can archive iCloud content before you downgrade to prevent the loss of data.
