I cannot turn off voice over on my husband’s iPhone 4s even after trying every action suggested. I cannot get off the home screen. I’ve tried swiping with 3 fingers, pressing the home button 3 x quickly, holding the home button down for 3 long seconds, etc. Sometimes I can get to where it asked for the passcode and I tap each number 3X as suggested but nothing works. Any other ideas would be appreciated!

How are you trying to enter the passcode ? You need to tap the number that you want to type so that it gets selected and gets the box around it, and then double-tap it to type it, not just quickly tap it three times.
Or if you have Siri enabled you could try using that to turn it off.
