I deleted and re-downloaded my apple music app on my phone, but I lost all my music and playlists after re-downloading it. How do I get my music back?

To restore your Apple Music subscription playlists:
in settings- iTunes App Store - your name - sign in
In Settings > Music, - turn on iCloud Music Library and "Show Apple Music”

If you haven't performed a sync recently, then you have simply lost the music purchased on the iPhone. So, keep resynchronizing it periodically, incase this were to happen again; that way you will have the option to retrieve it all.
