I can only listen to my recordings through the phone’s internal speakers, even though I’m connected to Bluetooth headphones which were working just fine right before I hit the playback button on the app. I see this has been an ongoing issue since at least 2014. Any ideas why or how to resolve this?

Hmm, it should work. Try this:
1) Open the Control Center
2) 3D press (or long press) on the music controls
3) Select the output option (little up-arrow with circles)
4) Select your bluetooth headphones
