Don't wait until you think you might need a back up. Back up regularly.
Given how easy it is to automatically back up your device using iCloud, I'd recommend you back up every day. That way, if your phone is lost or stolen, or drops out of your pocket and falls under the wheels of a bus, you've lost very little data.

I would say that it is advisable to have a recent backup for the installation of any update/upgrade.
I always backup my devices before I do any iOS update.

Think of it this way. Did you ever have an update fail? If not, lucky you ... but, if you did, having a backup is a good "insurance policy" to have in place.
Me too. I NEVER do an iOS update on any device unless it has been backed up before doing the update.
