I purchased an item from a game (hearthstone) yesterday night 1.99£ And this morning I realised that they charged me other 20£ on top for a product that I didn’t purchase
this is the second time happening can someone explain to me what it going on?

Did you call Apple, as I mentioned above? Call them, and see what they have to say. They'd have charges and payments on record. I wouldn't share the screen of the computer, as they would see account information and such.
So, call Apple, and see what they have to say.

Contact Apple Support. They’ll know what’s going on with a an extra charge. They’ll see it on their end as well, letting you know if, in fact, something else was, or was not, purchased.
