With 13.0 how do I save an image to my camera roll from a text message?

When you get to that screen scroll down. The choices to copy, Save image, sign to Contact etc. are below the line you describe.

Click on the image - hit the icon at the bottom left - then save image.

yes I have it otherwise I wouldn't respond to you.

It should be under that , if you see an edit actions - click on that and enable the options.
