I am using iPhone 11 Pro Max with AT&T eSim and an Indian (Vodafone) nano SIM. I am trying to get to the SIM Application/Toolkit to configure the physical SIM. I can't find this anywhere. Searching the web, people seem to have been able to find this in iOS 12 under Settings>Mobile Data which is not something I see on iOS 13. Can someone help me find this? Thanks.

Assuming that the SIM is properly configured, then the option should be at Settings > Cellular.
Question: Did you, by any chance, ever use the same SIM in a device running iOS 12? If you did not, are you able to do that for testing purposes?
If you have used this SIM with a device running iOS 12 and the option appears, then you'll want to reach out direct to Apple Support for additional help on this: Contact - Official Apple Support.
If you are not seeing the option appear, or you do not have a device running iOS 12 that you can test it in, do reach out to Vodafone for additional assistance in setting up the SIM

It appears that you're having some issues setting up the physical SIM when using two different cellular plans and an eSIM for one of them.
Take a look at this article especially the part that mentions having to download an app for your carrier to enable certain SIM features.

Thanks but I don’t think that is quite what I am looking for. The SIM Toolkit/Application is something that is on the SIM itself to configure the SIM - not something that’s on App Store. The service provider (Vodafone India) app which is available on the Appstore doesn’t help with configuring the SIM.

Thanks for clarifying that. If the app you're looking for should be on the SIM itself and the SIM is provided by Vodaphone, then Vodaphone support will be your best source for additional guidance on this.

My question is where in iOS 13 Settings should this SIM application show up? For iOS 12, this shows up under settings>Cellular>Data.
