I am trying to setup an iPad as a radio replacement in my vehicle and want it to handle calls for me so I can go hands free. I have an iPad Air 2 wifi only and an iPhone XS. I can supply a WiFi router in the vehicle but it will not have internet access but will allow to iPhone and the iPad to communicate. Will this work? I don’t have the access point yet and don’t want to buy one if it will not work. Thanks!

The “Calls on other devices” feature is predicated upon the iPhone and iPad being on the same Wi-Fi network - or, if supported by your chosen carrier and associated airtime agreement, Wi-Fi calling. Wi-Fi Calling is a carrier feature that permits the iPad to make or receive calls independently of the iPhone, over Wi-Fi, sharing the iPhone cellular account.
To my knowledge, neither of these features are available to devices when tethered to iPhone’s Wi-Fi hotspot.
As described, you can’t use the iPad in the manner your describe. Also, be aware that the personal hotspot connection to the iPhone will automatically disconnect after a period of network inactivity - such as when the iPad sleeps.

This will not work the way you have described. Try and keep it simple, connect your iPad to your iPhone through Personal Hotspot.
When I use the hotspot from my phone and connect the iPad to it the calls do not go through to the iPad.
That is very strange. Unfortunately it sounds like this setup will not work for you. You would be better off installing a CarPlay enabled console in your vehicle.
