Since I updated to MacOS Catalina on my Macbook pro's audio streaming output via my bluetooth speaker keeps switching to the internal speakers despite indicating the speaker is still paired to the mac. No such issues of such kind while using Mojave. Any ideas?

I tried that and still had the issue. I was grasping at straws on the the Parallels. I just happened to notice that when running parallels but only when running the Music app. Seemed to stop when Parallels was not open. Thanks for letting me know though. I was hoping I wasn't crazy but thanks to you... LOL
I have never installed an audio program or virtual audio cable/device.
As for hands free, I only used my MB Air to play music via bluetooth because my iPhone was completely broken (powered off and not usable), so hands free is likely not the issue.

The thing is that we are all facing the same issue here :(
What I'm saying is that I had this issue on the Mojave OS as well and I was waiting Catalina to solve that bloody problem.

I am also experiencing this issue. After upgrading to Catalina, non audio bluetooth devices are connecting just fine (magic mouse, trackpad) but the audio devices are not:
Before installing Catalina OS, bluetooth worked fine with any device. It seems that only the audio output is being affected.
I've deleted the bluetooth file from Library/Preferences, rebooted, shut down, toggled bluetooth on and off nothing works. The problem keeps coming back making it impossible to use my audio peripherals..
I have this same problem - but it impacts wired audio sources. I typically have a USB connected mike and speakers. I've seen the switch happen in various apps.
Zoom is a good one as it displays a notice about the changes. Using browser tools for podcasting (zencaster) was a total disaster last week.
Same issue on Macbook Air (Catalina) with Sony wf-1000xm3. Very annoying...

Also upgraded to 10.15.1.
I was on a conference call for 3 hours without a drop. But the audio dropped listening to youtube music twice in the last 30mins.
I have updated to 10.15.1 - no change still experience the issue.
I thought deleting this did the trick as well. But after 2 hours of connection it went back to original issue.
