Problem with CarPlay phone calls
I have 2019 Lexus nx300 and iPhone 11 pro max. Recently when I have a phone call people at the other end complaining that they almost can’t hear me.

I see that you're having an issue with the volume on phone calls when using CarPlay. I'd be happy to provide you with some information to help.
First, let's make sure you're using the latest version (13.2.2) of software on your iPhone. Then unpair your iPhone from your car and pair it back and test to see if the issue remains. If the issue remains, you may want to reach out to your car manufacturer and see if there is a firmware update for the car.

Same issue on a 2019 Lexus ES350. Person on other end of the call continues to have fading out audio to the point continuing the call doesn’t make sense. When disconnecting the USB mid-call (going off CarPlay) and back to Bluetooth the audio is fine for the other person. Issue occurs on both iPhone 8 and XR both have always run the latest iOS (currently 13.2.3) and have been connected using certified cable. The Lexus firmware was last updated by dealer Nov 2, 2019 and no improvement/change with the issue. Doesn’t matter if person on other end is on CarPlay, Bluetooth in car or regular handset.
After latest update, the issue seems to be resolved. iOS 13.3.1
Same issue on a 2020 Lexus NX300. Person on other end of the call complains that my voice can barely heard by using Carplay connection. However, the audio is fine for the other person when the call is under Bluetooth connection. Issue occurs on both iPhone 11 and 11 Pro (both are running the latest iOS) and they are connected by certified cable during the Carplay operation. There is no solution/improvement/change with the issue from local Lexus Dealership (last visit on 16 Jan 2020).
I also have this problem...2019 Lexus NX300. I talked to Apple today and they said they are aware of the problem and their engineers are working on it. Great that they are aware of it and it isn't an unknown issue.
Same issue. I have a new iPhone 11 Pro, 2019 Lexus NX 300. Seems to only happen in CarPlay mode (Bluetooth works okay) and only if I call another iPhone. If I call an android phone with CarPlay they can hear me ok. Very weird.
Same problem, but not only with the audio in the car, but with ALL the phone calls. People keeps complaining they can't hear me and that the audio signal is choppy. All since the last software update. I have ALL kind of problems. Can't function with this device anymore.
Everything was working perfect when I had iPhone X.

I have the same issue, people "on the other end" CAN´T hear a word I´m saying on the phone!!! I have a new iphone 11promax :( What on earth happened on latest ios update ???

my problem is similar but it only happens when i am calling someone who is using an iphone 1. it sounds like i am talking into the handset from fara way.
Same issue with new update in a different car. Car play makes no phone calls to any phone type. They all get transferred to my phone.
They are working on this problem since November, 13.
