I bought Apple watch 5 from US, when I try to use ECG app is not working with my old iphone 7 plus which is bought from Turkey.
Getting an error, The ECG app is not available in your region.
Any solution without buying new iphone from US :-) ?

buying a new iphone from usa will not help
for ECG to work the watch has to have been purchased in a country where it's enabled AND! used in a country where it's enabled

so no matter what iphones you get the watch will not work with ECG until the day Turkey enables it

My friend bought new iphone 11 pro and apple watch 5 in last week from US, ECG app is working very well in Turkey.
