I see that you're wanting to add manual information to the Health app. I'll be happy to help out with this.
You can manually add information to the Health app by following these steps: Manage Health data on your iPhone, iPod touch, or Apple Watch - Apple Support. When you open the Health app and click Browse, you'll have several categories to choose from like Immunization and Medications, among others.
