Voicemail plays different messages ... frustrating

Voicemail seems to play a different message to a caller when I am busy on the phone, compared to the standard message that it plays if i don't answer the phone. (When the phone is busy it plays an old message that I thought was deleted & replaced by the new standard message)

How do I fix this?
