Not good, sounds like a failed HD. Try resetting the NVRAM per the directions in https://support.apple.com/en-us/HT204063 and if that does not work, you can try to restart in Safe Mode per https://support.apple.com/en-us/HT201262 and if that does not work, restart in the Recovery Partition (hold down Command + R on startup) and then open Disk Utility. In Disk Utility run First Aid several times and see if any errors appear, if they do than confirms the HD is damaged and should be replaced. Even if the report does not show errors and the problem is still there you should take it to your local AASP to be diagnosed and repaired.
