The Spotify Apple Watch app is just a remote control for the iPhone Spotify app.

Spotify app does not stream music to the Apple Watch.

Apple Music can stream to the Apple Watch
You can transfer music stored on your iPhone to your Apple Watch to be played offline (Podcasts and audiobooks as well).
Pandora paid customers can transfer an offline playlist to the Apple Watch for offline listening.
