All are iPhones unlocked now? For example if I bought an iPhone with T-Mobile carrier, but I changed to at&t will the new SIM card work ?

In the US, all iPhones are not unlocked. Unlocked iPhones are only sold at the Apple Store or by Apple on its website when the iPhone is paid in full during the time of purchase.

From your example, in your post, the T-Mobile iPhone will only work on AT&T (or any other carrier) if you request T-Mobile to unlock it first for it work on AT&T. In short, the iPhone can be unlocked by the carrier which originally made the sale. If you are moving to another carrier then ensure that your original carrier has unlocked your iPhone. Each carrier in the US has its own requirements that the customer has to meet before they unlock the phone. Get more information on this from your carrier's website or call them.
