Hi my I cloud is usa and am in Africa and yet my Ecg won’t work

ECG must have government approval in the country where purchased AND must have government approval in the country where it is used.

The Apple Watch when manufactured, knows where it is going to be shipped for sale, so that covers the country where purchased. And the Apple Watch has GPS, so it knows where it is being used.
If your country is not listed in the above link, then you have to wait for your country to grant approval.
