Two things...
1) Try a forced-restart of your iPad with the Lightning to SD Card adapter connected. Sometimes external storage devices connected via an Adapter are not recognised after an OS update - this not being an uncommon event after update to iPadOS 13.x. Once cleared, the problem doesn’t return.
2) With the Adapter connected, check the SD Card adapter model number (Settings > General > About > Apple SD Card Reader) and let us know. The earlier (original) device version is A1441 - the newer model supports USB3 on iPad Pro, has a slightly longer/thicker body; this is model A1595.

My reader is a new A1595 model.
I tried several restarts and forced restarts of my iPad, with no success. Then on about the fourth try, using the (Settings>General>Shut down) option, everything worked as it should on restarting the iPad with the reader attached.

When you say "Apple Lightening to SD card reader," do you mean the current "Lightning to SD Card Camera Reader"? There was an earlier model with lower specs that came as part of a "camera connection kit".
What are the specs (speed and capacity) on the SD card? It may be too large or too fast for the adapter you are using. (It might even draw too much power.)
I think the old one handled UHS-I cards up to around 32 or 64GB. The new one handles UHS-II cards of 128GB (and maybe higher).
Was this setup working for you before? What has changed since then?
If this is the first time you have tried this card in this reader, the card may be of a type or in a format not supported by this reader.

No, it's a reader for the SD card with a short cable (150mm?) to the Lightning plug. You have to take the SD card out out of the camera to use it. It's the current model Apple and other retailers are selling in Australia.
Now here's an interesting new development: I thought to try the adapter and the SD card plugged into my iPhone 6 rather than my iPad, and it works perfectly with the phone. Still no joy with the iPad. For ease of editing and judgement of photo quality, I'd prefer to use the iPad to review my photos while travelling.
