I keep being asked to enter the password for an old Apple ID that belongs to someone I used to Family Share with. But my account is already logged in. I’ve tried signing in and out of iCloud, turning Family Sharing on and off, and even doing a factory reset, but nothing’s working. I’m unable to make any purchases or do anything that requires an Apple TV password.

They wouldn't need to access the TV if you downloaded purchases shared by their ID when you were in Family Sharing. If you aren't in Family Sharing with the person any more, you need to delete the purchases that they made.
Do you have purchases on the Apple TV that were made by another family member? If you delete the purchases, does the request stop?
But I don’t think we ever shared anything other than Apple Music while we were Family Sharing—I never turned on sharing of app purchases. And we only Family Shared for a week or so.
(This same user was actually signed into the Apple TV at one point, but that was almost a year ago, and only briefly, so I doubt it’s related.)
I don’t think the other user even touched this Apple TV while we were Family Sharing, actually.
Update: An account has appeared on my Apple TV called “(null)’s Account”. Seemingly no way to remove it.
I’ve tried a factory reset with updates; “(null)’s Account” still appears. I haven’t yet tried a factory reset without updating to the latest version.
