It was purchased at Best Buy location already unlocked. I used it at a certain carrier then switched carriers and now the device is locked and the only carrier it was used under cannot unlock it. I was told I can change carrier settings through my iTunes account, but I cannot seem to find where exactly.
Best Buy does not sell unlocked phones. They sell Flex Reseller phones that start out unlocked and will lock to the first carrier the device is activated on. You cannot change any settings on the device, and if the carrier won't unlock it, then you are out of luck. If you took this device outside the US to use, it cannot be unlocked and activated outside the US. No one but the carrier the device is locked to can authorize an unlock.
I wish they didn't do it. It is like another retailer in the UK that says the same thing. It says unlocked, until it locks to the first carrier they put a SIM in. And from what I understand, the carriers in the US either can't or won't unlock them here, and they can't for use overseas, the way it has been explained to me. So many people from outside the US purchase them at Best Buy, and then then cannot use them.
Only your original carrier may do that - if they choose to do so.
